Ask HN: How do you keep track of the skills/tools/frameworks that you learn?

There's so much we constantly learn but do not keep track of. Over the years, we forget about it. Do you know of a flow to keep a chronological record of everything that you learn - could be a skill chart/list etc. in a presentable format.
