I worked on the build for the Nome Alaska (norton sound) hospital in Nome, AK.<p>There was a guy on their IT staff who legally changed his name to his SSN.<p>He went by the name "Two Six"<p>His identity had been stolen at somepoint using his SSN. The first two digits of his SSN were "26" -- and he went by that name.<p>His logic, apparently, was that if his legal name was his actual SSN his identity couldnt be as easily stolen... I never quite figured out his scheme...<p>However, after some time, I sent him a link to a jacket that had 26 pockets, and I told him I thought he should know about it. It seems the interesting part is requiring a criminal background checks <i>for parents</i> to attend a play.<p>Yet they’ll hand the same kids back to the parents every day.
The menu seems to suggest you can proceed without entering SSN and be a spectator. It's a dual purpose form for chaperones/drivers where background checks are probably necessary.
The form in the image hints that this is for a school in North Carolina.<p>“list any addresses outside of NC in the past 7 years”<p>Why does the title say Washington State?
