Vaughan <i>Challenger Disaster</i> book used that term to describe a specific bureaucratic process where hundreds of unacceptable risks in the Space Transportation System were "accepted" and managed for each launch. It has also been used to describe the process by which errors in the construction of nuclear plants are analyzed and determined to be critical, trivial, or something in between.<p>More frequently though people use it to describe the non-process through which doctors go into surgery without washing their hands or forklift operators get in the habit of smoking pot at work.
