I'm interested in putting together a moderately complicated survey, and am pretty shocked at the apparent dearth of options. Scanning Google and Github didn't pull up very much promising software:<p>* http://www.limesurvey.org/<p>* http://www.surveyproject.org/<p>I'd self-host. The survey itself would have multiple-choice questions which can branch/skip and some free-choice sections. It needs reasonably customizable styles so it doesn't have to look like it's from 1998. Easy-to-implement iframe embed-ability would be a big plus.<p>Honestly, I could roll something like this myself in a week, which is why I figure Someone Must Have Done It Already. But who?
