Great start!
I used to work at Typeform, so always fun to see these alternatives popup :)
It seems like a good beginning, but missing some polish, especially around keyboard usage. You should probably allow users to finish an answer by pressing "Enter" and it goes to the next one, Tab + Shift+Tab goes to next/previous question and so on.
The rating field also seems to return "This field is required" when you put in something else than numbers. If you use <input type=number/> you basically get free validation :)
As a question for the service/business itself, how do you plan to differentiate yourself from Typeform? As it stands right now, Typeform would probably beat you on design/UX, so you might want to figure out a different angle to attack.
Good luck!

There are so many form builder tools out there in the market like the one you mentioned which is Typeform, another one is Jotform, and recently there has been another new product called neetoForm, which is also priced better.
I saw you mentioned that pricing would be low and you are focusing on the UX. But neetoForm has even better pricing when I checked yours and their annual plan. I agree that your 399$ lifetime access is better but isn't it just a limited-time offer?
So just curious to know how do you plan to compete with all these products.

How accessible are the generated forms and the form builder itself? Has it been tested for accessibility?
